Access Card Create

Form PAM-CK-001 — personnel access credential for PAM database access

This terminal issues the access card key required for Central Accounting portal access — your personnel identification for the internal Personnel Asset Management (PAM) interface. Through PAM you may view assigned Foundation Credit balances, canteen deductions, and compensation line items. The card key is not a physical credential until processed by Personnel.

All personnel must issue an access card before using portal forms and records. After you verify the preview, you may download a personal copy (PNG), file the card with Personnel Records via HR intake, or do both. HR filing is optional; card issuance is not.

After card creation
  1. Complete the fields below and verify the card preview.
  2. Personal copy (optional): Download the card key (PNG) for your own records, profile, or wallet background.
  3. HR filing (optional): Use Personnel Records filing under the preview. Attach the PNG manually when posting on X or Reddit.
  4. Use the comment field for a personal motto or stated goal. Do not enter workplace grievances, canteen complaints, or similar remarks.

Personnel may supply a voluntary likeness for access card embossing. Unauthorized likenesses are prohibited. Do not upload images you do not own, copyrighted material, official identification documents, or photographs intended to impersonate another individual, Foundation officer, or real-world person. Violations may result in immediate credential revocation and a memo you will not enjoy reading.
